If you have insurance in the above list, you also can contact us before you come. We can help you to check your insurance benefit and coverage. You have to provide the followings information:
1. Your full name.
2. Your date of birth.
3. Your insurance company’s name and number ID (on your insurance card).
4. Your insurance provider’s phone number (on your insurance card).
5. Your contact phone number.
We will let you know how many visits your insurance company allows, how much your co-pay and deductable will be, etc.
